I enjoy having nice sets, even if its hardly noticable, the small difference makes you feel unique, especially if it's a custom set you've made. And I'm sure its effective as Valve pulls in millions monthly from these sets.
What do you expect when that is exactly how the game is played as you pointed it out yourself?
There are some items which can make a hero appearance to be so different from its default outlook that you might confuse it with other heroes, especially those changing the color theme of the hero or even just the helmet alone. I would want to suggest Valve to create a mode which allows a player to view all 10 heroes in their default appearance so the confusion would be eliminated.
Of course, that is a subjective view for the degree of confusion between heroes.

For a noob nerd that only plays 1 hero like me, it does provide me with some variety wearing different mixed sets each individual game....
I love puck's reminisce of dreams set its one of the best made so far although they forgot they custom icons that shouldve come with it
I notice them a lot, usually because stuff I've got is fairly notiuceable, legion commander's banners are visible from a mile away, sword is fucking huge and helmet is visible on the portrait, even shoulderpads are very visible in the game. I also have 3 distinct Sven sets and each one of them looks very unique in the game (iron drakken, chiseled guard, and fiend cleaver are all very distinct, and I have some capes from other sets to mix it in with it, as well as some swords). It looks really good and really noticeable.
Also I j8st might get one of the spectre sets cause they are just so fucking badass.
I love pressing i and watching my hero walk towards the rune looking badass in his fancy outfit. Or just looking badass in his default set because poor Phoenix still doesn't have cosmetics.
BTW the Spectre Voice Immortal sounds pretty cool and its cheap (or was when I got it).
I notice and other peoples stuff but only like at the start of a match, once it's going and you're in the heat of battle it's hardly noticable. I barely notice what spells people are casting in a fight much less their armor.

I get that everything has to be in line with guidelines etc and its more difficult to design dota cosmetics, but yeah its so difficult sometimes to actually see a difference
i like my meepo immortal, hat and gansta set because it makes you feel like pro even though im a 1k pleb that barely plays ranked
I've got a 9 win streak on Zeus since I bought the immortal item, so yeah it makes a difference I feel.
Not really a fan for cosmetic unless it changes their spell animation, such as Disruptor Immortal with his Kinetic Field :D

I got a couple awesome looking sets recently and thought I'd be enjoying them when playing on those heroes... but it turns out I don't even notice a difference past the pick screen.
