When the team gets too cocky and tries to start fights 2-3v5 and end up losing several fights that could be avoided. Or just single heroes looking for pick-offs alone near the whole enemy team and end up feeding.

You can only throw if you are ahead in the first place. After that it's any dumb decision. It can be refusing to push/take rax after winning a team fight and then enemy team comes back late (common). It can be diving rax when you are way ahead and can just wait for rosh, or split push the lanes up/farm a bit more til you're strong enough (most common throw imo). Or it can be just playing stupid, getting picked off when you really shouldn't, not defending when you should, whatever.
Clearly winning and then 1 minute of ultra cockyness! Buy out on every hero and then proceed to go into a fight or different fights 1 by 1 (Get owned) and then watch your opponents rush up 1 lane, kill 1 tier 3 onto tier 4's and throne because you finishing your mkb was more important then buy back. This normally happens when you probably a rax or 2 up. #HowToThrow

and also when you are muted and can only ping on the fucking t4 tower 3 fucking times, and watch them tp, and watch your mmr go down.

Buying a divine rapier and giving it to the enemy team. I had a game where I was Treant and our Drow Ranger bought a rapier and ended up giving it to an Elder Titan. We lost.
